Cyber Monday Contest Form

Holding an account is not required to enter. One entry per hour.  To enter, follow instructions as stated.  Application must be received by the deadline date stated. You will be automatically entered when you fill out the registration form. Giveaway begins on 9:00 am Monday the 28th. Closing date of this Sweepstakes is 4:00 pm Monday the 28th. You must be 18 years of age or older to enter. Each entry has an equal chance to win. Entries received from persons residing in geographic areas in which entry is not permissible will be disqualified. First Central State Bank is not responsible for lost, late, non-delivered or misdirected email. Grand Prize is (1) Apple iPad 2 and will be awarded at the end of the contest period. The prizes may not be exchanged for cash. Winner is selected in a random drawing on, Wednesday the 30th. Approximate numerical odds of winning are dependent upon number of entries. Sweepstakes open to legal residents of the United States, its territories and possessions. Employees and Volunteers of First Central State Bank, or members of their immediate families are not eligible. Winner will be notified following the drawing.  The laws of the USA govern the sweepstakes. All federal, state and local laws and regulations apply. Void where prohibited. Any prize notification returned as undeliverable may result in disqualification and selection of an alternate winner. First Central State Bank reserves the right to disqualify any entrant if the contest rules have been violated in any way. By entering, all participants agree that First Central State Bank may use their name, photograph and likeness for promotional purposes. All qualifiers agree to provide First Central State Bank their name, address, phone number, and social security number. This will ONLY be used for processing necessary information as it pertains to prizes. Prizewinner agrees to waive any claim of royalty, right of further remuneration for any such promotional use.
